Insurance Plans We Accept
We accept payment for your visit from your insurance carrier with the balance due to be paid within 30 days. We require the deductible, copay, co-insurance payments and non-covered fees be paid at each visit. In the event of duplicate payment you will be reimbursed.
We accept the following insurance plans; however, please call your insurance company to verify your coverage and see if we are in-network. We do not accept Medicare, Medicaid or Oregon Health Plan.

Your first visit in our NW Psychiatry office requires payment in full 1 week prior to the appointment. Subsequent appointment payments can be paid at time of service with checks, credit cards, or HSA/FSA cards. No cash payments accepted.

If you have billing questions, please ask the office staff, as Dr. Salbador doesn't deal with billing.

We charge a missed appointment fee if there is less than 48-hour reschedule notice. Please read the FAQ’s for more specifics.
